Il nous faut arrêter de parler du patient « non observant » à ses traitements mais plutôt d’un patient présentant des difficultés d’adhésion médicamenteuse

Résumé

L’adhésion médicamenteuse peut se synthétiser sous la forme d’un rapport entre « ce que le patient fait » et « ce que le patient et le médecin ont décidé après négociation sans imposition ». Cette définition doit être mise en contexte avec le moment de la maladie et son vécu par le patient ; les actions d’accompagnement qui en découlent doivent être adaptées et négociées avec le patient tout au long du processus de prise en charge. Cet article propose de modéliser ce parcours et offre des options pour la pratique courante. Les éléments clés qui en découlent pour la pratique du pharmacien sont : simplifier la prescription ; communiquer de manière pertinente et adaptée au stade d’acceptation du patient de sa pathologie ; se former en accompagnement motivationnel.

Summary

Medication adherence can be defined as the link between “what the patient implements” and “what the patient and his doctor have decided together after negotiating without constrains”. This definition should be put into perspective with the chronology of the disease and the way the patient experiences it. Counselling actions should always be adapted to the situation and negotiated with the patient, all along the process of care. This article proposes a model for this process and offers options pour tailored counselling. Key elements for pharmacist's practice are: simplify the prescription; communicate with the patient according to his stage of acceptation of the disease; get adequate training for motivational interviewing.
